a) Online Registration: New applicant can register himself as a distributor of Pueraria Ayurveda Private Limited via online www.puerariaayurveda.com by using following steps:

1. Applicant will get the referral code to his /her mobile number referred by his/her sponsor which will remain valid for specific time.
2. After getting the referral code applicant can fill up the application form through Register section of the Pueraria Ayurveda website.
3. Applicant can access application form by using combination of referral code and mobile number and fill up the details provided in the application form.
4. After filling up the application form applicant will have to accept Term and Condition of New application and Agreement form and submit.
5. After submit, the application form will be in approval stage and applicant can login from Applicant Login section to check application status.
6. After 1st stage approval applicant will have to submit hard copy of self-attested KYC with required documents to the Head Office within 45 days from the date of 1st stage approval.
7. After submission of hard copy of KYC the applicant will have to wait for final approval and then after getting the final approval he / she can be a Distributor of Pueraria Ayurveda Private Limited.

b) Offline Registration: Applicant can collect offline form of Distributor Application cum Agreement Form from our Head Office or from any of our Distributor / Service Point. He shall fill up and submit the Distributor Application cum Agreement form with required self-attested documents at the Head Office or in the office from where the application form was collected.
